Being the physical host to an Outer God or a Great One has those consequences. You immigrate to Upper yharnam to get finally get away and start a new life but then the beast plauge begins. You then decide to to harness your inner power of blood, arcane, and beasthood to not only survive but also end the plauge once and for all. Where the story goes from here is up to you.
