Murakumo with Darkmoon blade build. If you want to go 76 poise instead of 56, Id take 6 points out of DEX and add them to END.Then switch your chest to knights chest ect. Mothers mask also optional, just used it to supplement health.
Murakumo is obtained from killing Shiva of the east or the Giant Skeletons. The Giant skeletons near firelink CAN drop them (I just farmed one) but it can take awhile to drop (3 hours for me :/)
30 faith is just for darkmoon blade miracle, and theres only enough attunement for 1 spell slot. this is another place you could add points (6 from END) if your happy with a 56 poise build.
also use highest adjust talisman you have, i just threw one in the planner to check weight. :D
