The primary weakness in most heavy armors, other than their crippling weight, is their lack of magic defense. This build is designed primarily to circumvent this defense.
With bare minimum physical scaling stats and softcapped faith, the Divine Zweihander is an excellent choice for smacking someone in their weakspot due to the high amount of magic damage it receives when taken to the Divine path, plus it is backed up by the very much powerful yet swift Wrath of the Gods. In the off-chance someone prepares with Great Magic Barrier, the Shotel is lightweight, fast, and can be buffed with lightning (the element which armors and shields provide the least defense against) to be as strong as the Divine Zweihander. Of course, this buff is timed, so once the Sunlight Blade fades away, you can either keep using it against a turtling opponent, or switching back to the Zweihander and continue doing high damage. Dragon Crest Shield makes up for your lack of fire defense, and you're going to have to fast roll through all those lightning spears and zweihanders.
Outfit chosen for appealing form. If you want to tip the scales toward function, use Mom Mask and Black Iron Gauntlets and Leggings to meet 36 poise and 1945 HP without any change of stats.
