For fists only (Mostly). The surprise is karmic justice, BUT you need to switch to the pyro hand before it goes off. If you do this the damage inflicted by karmic justice will be massive, at least 700+ damage once it activates. Another note is the pyro hand needs to be in the right hand at the time of activation, not casting, for it to work. Can also replace brigand hood with MoTM while still fast rolling if you feel like you need the extra health. Enjoy.
