Quality build
Other notes:
For those who don't know a quality build is strength and dexterity combined together hence the name jack of all trades as a saying for the best of both worlds.
Claymore is probably the best weapon for this type of build as is has the best scaling for both stats equally (not specialising in one or the other so you aren't wasting any points).
The murakumo is just my favourite personal choice but you can use any strength / dexterity / quality weapon instead (such as the halberd or bleed weapons for extra damage if you want extra damage or a strength weapon if that's what you prefer).
Swapping the mask of the mother for the helm of thorns may reduce practicality but does look quite nice.
Any secondary ring will do, I just chose the wolf ring for a bit of extra poise.
